Development of European standardised glaucoma datasets and adaptation of glaucoma terminology for electronic medical records and teleconsultation systems

Objective: The increasing digitalisation of medical records and the possibility of data exchange by linking different diagnostic systems allow multiple options, which are commonly not used in practice. In particular, the glaucoma patient with a long disease history can benefit from long term storage of the findings related to his/her glaucoma. It is therefore necessary to develop standardised terminologies and data sets for long term data storage in glaucoma patients.
Methods: IT based projects related to ophthalmology (OPTHEL, Diab-Card, DOAS) were analysed. An optimised multilingual dataset focussing on clinical eye care for the documentation of glaucoma related findings was developed.Using datasets and data exchange of a cohort of 14 glaucoma cases between 7 specialised European glaucoma centers, the efficiency in data evaluation and the possibilities of teleconsultation were evaluated and verified.
Results: Sample-datasets and input forms for data collection are presented. To provide an easy approach for translation of the data set items into different languages, free text fields were avoided where possible. By using existing terminology (European-Glaucoma-Society Guidelines) the ICD codes related to glaucoma were enhanced without losing downward compatibility.
Conclusions: The development of standardised European documentation for terminology and datasets for glaucoma related findings is the basis for future quality management and teleconsultation. By evaluating sample datasets from different glaucoma centers a teleconsultation system could be successfully simulated. In a further step disease management programs (DMP) for guiding decisions in glaucoma treatment can be added to the system.
