Anticipating and managing Intraoperative Floppy Iris Syndrome (IFIS) could assure successful cataract surgery because, according to DF Chang, "IFIS increases the risk of complication when it is not anticipated. Obtaining the right information regarding past and present intake of alpha-blockers is mandatory, because advance knowledge of alpha-blocker is essential in reducing complication. Patients considering cataract surgery should be screened for these medications, particularly if their pupil dilates poorly", according to the AAO/ASCRS Joint Alert 2006. A postoperative iris sign is described, which helps with history taking before surgery of the fellow eye; 3 cases of IFIS correlated to a local (not systemic!) use of alpha-blocker (Dapiprazole drops) are reported; the rules for iris preparation are listed. |
