Frequency of paracentesis after intravitreal Avastin injections: comparison between 27 gauge and 30 gauge needle

Objective: Intravitreal Avastin injection (0.1 ml, 1.25 mg bevacizumab) may lead to a significant elevation of the intraocular pressure (IOP). A paracentesis may become necessary for acute reduction of the IOP in certain cases. The purpose of this study was to determine 1) the frequency of paracentesis performed and 2) the grade of reflux under the conjunctiva depending on the thickness of the used needles.
Methods: Retrospective review of the surgery reports of 210 consecutive intravitreal Avastin injections in 138 patients between January 2006 and March 2007. In particular, the frequency of paracentesis, the used needle thickness (27 ga versus 30 ga), the lens status and the semi-quantitative reflux grade were analyzed.
Results: A paracentesis was performed in 68 cases (32%) for acute reduction of the IOP. In the 27 ga group the frequency was 26% (25 of 96), in the 30 ga group 38% (43 of 114) respectively (p=0.08, Fisher’s exact test). In 36% of the cases (76 injections) an intraocular lens was found. The medians of the semi-quantitiatve assessment of the reflux grade under the conjunctiva were 2.0 in the 27 ga group (corresponding to a mild reflux) and 1.0 in the 30 ga group (corresponding to a minimal reflux).
Conclusions: When using a 30 ga needle for intravitreal injection of Avastin the reflux under the conjunctiva is less compared to a 27 ga needle. However, use of 30 g needle appears to be associated with a higher frequency of paracentesis.
