Modified microperimetry (MP) using Scanning Laser Ophthalmoscope (SLO) in choriodal neovascularisation (CNV)

Central visual field defects in patients with CNV and anti-VEGF therapy can be detected using static SLO microperimetry with manual tracking but modified strategies and modified evaluation. Foveal position was detected by SLO funduscopy using argon illumination, CNV borders were drawn from fluorescein angiograms. Simple strategies beginning in the peripheral area gave high densities of measured retinal sensitivities wich could interpolated to result in a pattern like distribution. Thus, deviation and stability of fixation, mean sensitivity in the CNV area and within areas centered around the fovea or fixation center were evaluated. In 50 patients it could could be shown that after 1 year anti-VEGF could stabilize visual acuity and fixation as well as the mean sensitivity in the CNV area. However, recurrencies were only detected under favourable circumstances. In addition, it could be shown that a more precise detection of scotomas can be achieved using real-time-tracking instead of manual tracking (Reinhardt, J.). As a consequence, more patients with unstable fixation can be examined in future times. The examples show that SLO-MP together with visual acuity, angiography and OCT is helpful to estimate the prognosis before therapy and to improve the evaluation of important parameters after therapy (changes in sensitivity areas in relation to the location of fovea and fixation area). In conclusion, it seems to be possible to improve the strategies for anti-VEGF therapy using a modified SLO-MP as additional examination.
