Validation of a new mechanical device for measuring the horizontal anterior chamber angle intraoperatively

Objective: Phakic intraocular lenses as an alternative to refractive corneal surgery are getting more and more in the focus of interest. In particular, for correction of higher myopia and partly hyperopia several angle-supported anterior chamber phakic intraocular lenses are available. To avoid related complications such as pupillary block, pupil ovalization or increase in intraocular pressure, a precise preoperative measurement of an anterior chamber distance from angle to angle is necessary.
Methods: 20 eyes of 20 cataract patients were examined preoperatively with the SL-OCT manufactured by Heidelberg Engineering. The horizontal angle to angle distance of the anterior chamber was measured. Afterwards, a measurement of the angle to angle distance was done during cataract surgery before performing the capsulorhexis. The measurement was repeated three times by the same operator.
Results: The measurement of the angle to angle distance could be successfully completed as well with the SL-OCT, as with the new mechanical device. The mean distance measured with the SL-OCT was 11.64±1.01 mm compared with the mechanical device in which the mean angle to angle distance of 12.12±0.76 mm was found.
Conclusions: By comparing the SL-OCT and the mechanical device there was a slightly wider horizontal angle to angle distance measured intraoperatively with the latter. The new mechanical device is a cost-efficient, uncomplicated opportunity for precise measurement of the angle to angle distance of the anterior chamber before Implantation of angle-supported phakic intraocular lenses.
