Pars plana vitrectomy in macular pucker: Comparison of cases with and without ILM peeling

Objective: Peeling of epiretinal membranes have become a standard procedure in vitreoretinal surgery. Preliminary data raises the question whether additional ILM Peeling can reduce the rate of recurrences.
Methods: 283 patients with macular pucker treated in the Department for Vitreoretinal Surgery, University of Cologne, were included into the study. All patients underwent pars plana vitrectomy. In 89 cases additional ILM peeling was performed supported by indocyanine green colouring in 76 patients. Anatomical success was decided upon funduscopy during the follow-up of 6-12 months.
Results: In the group which underwent additional ILM peeling 88 out of 89 patients (98,9%) had anatomical success. Supported ICG colouring led to a success in 75 out of 76 cases (98,7%). The group without additional ILM peeling showed a success rate of 188 out of 194 patients (96,9%). Differences were statistically not significant.
Conclusions: According to our retrospective data, ILM peeling and ICG colouring does not seem to have any additional positive effect on the outcome of macular pucker surgery. Long-term follow-ups in randomised clinical studies will be required to further evaluate the role of ILM peeling in macular pucker.
