AbstractP 218 EpiLASIK topography-guided after treatment for the correction of irregular astigmatism Hufendiek K., Herrmann W. A., Prahs P., Helbig H. University Eye Clinic, Regensburg Objective: Topography-guided refractive surgery with an Excimer Laser is used to treat irregular astigmatism. The objective of this retrospective study was to judge whether this treatment form is safe and effective in eyes that have previously been subject to Excimer Laser refractive surgery. Methods: 29 eyes of 23 patients with irregular astigmatism after primary surface ablation underwent topography-guided (WaveLight Topolyzer) retreatment in EpiLASIK technique (Keratome: Epivision, Gebauer, Germany) using the WaveLight Concept 500 Hz excimer laser system. For 3 days after photoablation, a soft bandage contact lens (Pure Vision, Bausch & Lomb) was applied. Pre-operative BSCVA, UCVA, SE and cylinder as well as corneal indices ISV (Index of Surface Variance), IVA (Index of Vertical Asymmetry), IHA (Index of Height Asymmetry) and ABR (Coefficient of Aberration) were compared with 3 months post-operative measurements. Results: Mean BSCVA improved from 0.06 logMAR (SD±0.1) to 0.03 logMAR (SD±0.1). UCVA increased from 0.36 logMAR (SD±0.19) to 0.16 logMAR (SD±0.18). SE was reduced from 0.26 D (SD±1.46) to 0.13 D (SD±0.91). Astigmatism was reduced from -0.89 D (SD±0.55) to -0.33 D (SD±0.25) in the main axis. Corneal aberration and asymmetry decreased also: The ABR decreased from 1.46 (SD±0.33) to 1.26 (SD±0.40), the IHA from 19.3 (SD±12.3) to 15.3 (SD±8.6). Conclusions: The results of this case series indicate that topography-guided retreatment is safe and effective in correcting residual refractive error and reducing irregular astigmatism after Excimer Laser refractive surgery.
