Efficiency of the lamellar and penetrating keratoplasty in patients with herpetic keratitis

Objective: To study efficiency of the lamellar and penetrating keratoplasty at patients with herpetic keratitis depending on a stage and features of course of disease.
Methods: The electronic database in which 196 patients with herpetic keratitis by whom it was made 288 keratoplasty, including 178 lamellar, 70 penetrating, 40 biological coverings by Puchkovskaya has been created.
Results: At patients with outcomes of herpetic keratitis in a stage of remission lamellar keratoplasty was effective in 100% of cases, penetrating – in 88 % of cases. In 12% of cases after penetrating keratoplasty transplant failure is marked. In a stage of recurrence of the disease the therapeutic effect was received in 97,9% of cases at lamellar keratoplasty, in 85% at biological coverings by Puchkovskaya, in 80% at penetrating keratoplasty. Results of lamellar keratoplasty are significantly better in comparison with penetrating keratoplasty and biological coverings by Puchkovskaya. The dynamics of visual acuity after lamellar and penetrating keratoplasty is different. The visual acuity directly after operation is higher after penetrating keratoplasty. In long-term observation the increase of visual acuity is more marked after lamellar keratoplsty. There is no significant difference of visual acuity in long-term observation after lamellar and penetrating keratoplasty in stage of remission as well as in recurrence of herpetic keratitis. In remission directly after penetrating keratoplasty visual acuity is (0, 42±0,05), after lamellar – (0,27±0,06), in long-term observation-(0,48±0,06) and (0, 46±0,07). In recurrence – directly after penetrating keratoplasty – (0,19±0,03), after lamellar – (0,12±0,03), in long-term period (0,26±0,12) after penetrating and (0,25±0,04) after lamellar keratoplasty.
Conclusions: The lamellar keratoplasty is method of choice in comparison with penetrating keratoplasty at patients with herpetic keratitis in stage of remission as well as in recurrence period.
