Tumor as a differential diagnosis in perimetry defects of glaucoma patients

Objective: Glaucoma is one of the main causes of blindness. Perimetry defects and atrophy of the optic disc is not always due to fluctuating of ocular tension. Apart from disturbance of perfusion you should keep in mind compression of the optic nerve.
Methods: Authors present two patients (1 woman: 78 y. and 1 man: 58 y.) who came to our department for excluding glaucoma. Whole ophthalmological examination including pachymetry has been done.
Results: Vision decreased during the last 6 months continuously in both patients. The 78 year old woman suffered from perimetry defect L>R and a difference of cup ratio of both eyes (CD r 0.6 and CD l 0.9). The tension did not show any peaks during the day and was stable. The 58 year old man turned out to have a slight vision field on his left eye and a defect of temporal vision field of the right eye. No eye suffered from tension peaks. For excluding any compression in the orbit we decided to perform a radiological examination. The CT showed a meningeom of os sphenoidal in one case and adenoma of Hypophyse in the other case.
Conclusions: Perimetry defects are not always caused by variable ocular tension. We recommend to do radiological examination if the ocular tension is stable and does not cause perimetry defects.
