Development of astigmatism and visual acuity after penerating keratoplasty with different trephine systems

Objective: Penetrating Keratoplasty (PKP) is an established surgical method with high chances of success. The amount of post-keratoplasty astigmatism after PKP is still the subject of numerous studies. The purpose of this retrospective study was to determine differences of post-keratoplaty astigmatism and visual acuity depending on the trephine system (GTS vs. motor trephine).
Methods: For various corneal diseases a PKP was performed in 74 patients (time period from 2001 to 2002). In 53 patients the GTS, in 21 the motor trephine (MT) was used. The astigmatism was determined at the following time points: preoperatively, postoperatively, on dismissal day, after suture removal and 1 year after suture removal.
Results: The preoperative mean astigmatism (A) did not show a significant difference in both groups (p=0.21). Immediately after PKP the mean astigmatism was 1.8±1.5 diopters (D) with GTS and 2.0±2.6 D with MT. After suture removal GTS 2.2±1.7D, MT 3.7±2.3D, one year after suture removal GTS 2.3±1. 5D, MT 4,0±2.0 (p=0.001). The preoperative visual acuity (VA) did not show a significant difference (p=0.261). At dismissal day GTS 0.2±0.13, MT 0.1±0.15 (p=0.312), after suture removal GTS 0.4±0.32, MT 0.3±0.21 (p=0.111), one year after suture removal GTS 0.5±0.30, MT 0.4±0.31 (p=0.433).
Conclusions: Due to the smaller postoperative astigmatism after PKP with the GTS a better visual rehabilitation is to be expected in contrast to the motor trephine.
