Laser thermokeratoplasty for presbyopia solution in hyperopic patients

Objective: To evaluate the effect of laser thermokeratoplasty (LTK) with a contact infrared laser in hyperopic patients of presbyopic age.
Methods: The infrared Ho; Tm; Cr, 2,12 wavelength YAG laser was used to correct hyperopia (manifest spherical equivalent of +1.0 to +4.5 diopters [D], manifest cylinder of 1.5 D or less) in 54 eyes of 27 patients with presbyopia symptoms (mean age 51,0±6,27). Laser pulses were delivered to the cornea for 0.5 seconds in 2 concentric, radially placed 12-spot rings 6.0 and 7.0 mm in diameter. The laser power was 130 to 180 mJ.
Results: The pre-op uncorrected visual acuity (UCVA) at distance was 0,47±0,22 and at near was 0,14±0,06. In 1 week post-op the distance UCVA was 0,17±0,08, and the near UCVA was 0,86±0,08, the mean correction was -3,30±0,73 D. The reduction in mean spherical equivalent was revealed during the next periods, thus increasing the distance UCVA, while decreasing the near UCVA. At 1 month the mean refraction was -2,66±0,67 D, the distance UCVA was 0,27±0,16, the near UCVA was 0,71±0,15. Stabilization of the refractive effect was achieved at 6 months: the mean spherical equivalent was -1,66±0,58 D, the distance UCVA was 0,41±0,08, the near UCVA was 0,73±0,12. The changes during the next periods were statistically insignificant In 1 week post-op the mean contrast sensitivity and glare-test were found to be reduced. Restoration of these findings occurred at 3-6 months post-op.
Conclusions: LTK is a safe and effective method of presbyopia correction in hyperopic patients.
