Depth of focus and evaluation of efficacy of accommodating IOL

Objective: Functional ability of accommodating IOLs consists both of the active part which occurs due to a change of the refractive optical power of the pseudophakic eye and the passive part determined by the phenomena of depth of focus and pseudoaccommodation. Purpose. To evaluate depth of field/depth of focus (DOF) and pseudoaccommodation amplitude (PA) in patients after implantation of conventional monofocal PC IOLs.
Methods: 93 patients (93 eyes), aged 52.6±13.3 years (mean±SD), who underwent phacoemulsification with implantation of PC IOL Acrysof SA60AT were included in the study. Average postoperative follow-up was 6.8±1.2 months. DOF and PA were studied by a devise allowing to measure VA at distance from 3 m to 0.2 m in every 0.1 m with the best correction for distant vision and +0.75 sph in addition. An interval of distance with maximal value of VA was considered as a DOF. An interval of distance with VA of 0.7 or better was used for calculation of PA.
Results: In eyes with BCDVA 1.0 (n=64), 1.5 (n=21) and 2.0 (n=8) DOF was 1.12±0.03 D, 0.62±0.02 D and 0.43±0.03 D respectively, PA was 1.12±0.03 D, 1.75±0.05 D and 2.24±0.07 D respectively (mean± SE). Average pupil size (mean± SE) was 2.9 ± 0.01 mm (from 2.8 to 3.2 mm).
Conclusions: PA depends on visual acuity, pupil diameter and angular size of test objects. Our findings on depth of focus and amplitude of pseudoaccommodation may be used for clinical evaluation of accommodating IOLs.
