Early outcomes after Ahmed glaucoma valve implantation by young patients with complicated glaucoma

Objective: Young patients with complicated glaucoma have a lower success rate after a filtration operation again because of a higher wound healing. We have implanted a Ahmed glaucoma valve in selected cases.
Methods: We implanted 8 Ahmed glaucoma valves in patients under 18 years of age. The average was 11,9 years (2,0-17,0 years) with a mean follow-up period of 9,4 months (1-15 months). Four patients suffered from congenital glaucoma, three from secondary glaucoma after congenital cataract operation and one from secondary glaucoma after trauma to the eye. All patients had a mean of 4,3 glaucoma operations previously.
Results: Praeoperative intraoculare pressure (IOP) was reduced from a mean 33,0 mm Hg (26-44mm Hg) to postoperative 8,3 mm Hg (2-22mm Hg). On the last visite the IOP was 18,4±4,3mm Hg (13-27mm Hg). During follow-up 5 patients needed revisions. In three cases a thick capsule of fibrotic tissue had formed around the drainage device. In another case fibrotic tissue has occluded the valve mechanism due to retrograde ingrowth. One patient needed a revision because of a tube dislocation.
Conclusions: IOP was successful lowered in most young patients, however half of the patients needed revisions due to early encapsulation.
