AbstractDO.24.10 Filatov's stepping stem in ophthalmooncological surgery Maletskiy A. P., Chebotaryov Y. P. The Filatov Institute of Eye Diseases and Tissue Therapy, Odessa, Ukraine Introduction: After removal of the tumour of organ of sight not always it is succeeded to remove arising defects up by local fabrics or transplants from their insufficiency or absence of sufficient trophism. In such cases as the method of choice the round stepping stem offered by V.P. Filatov in 1914 can be used. Purpose of work: to study possibility of application of Filatovs stem in ophthalmooncological surgery. Methods: Essence of method is in that on a front abdominal wall or neck and underjaw region a dermic-fatty stem is formed as a «suitcase handle». Every three weeks the next step is made by cutting off from distal end, by the turn of him toward a defect and fixing in soft fabrics. Good trophism of stem allows attaining repair of his fabrics on areas with an insufficient trophism, to form anatomic structures. A method was applied in two cases. First patient was in age 60 years after exenteration of orbit and excision of tumour of parietofrontal and temporal regions on an occasion basal cell cancer with regional metastases. Appearing defects were partly closed by local fabrics and dermic autotransplantate. Two Filatovs stems were formed in a underjaw region for closing of remaining defect of frontal and supraeyebrow regions, bridge of the nose and skate of nose with maximal long 12 and in thick 1,5 cm. To the second patient in age 23 years, was formed the stem on a front abdominal wall to form the eyelids and conjunctival cavity, after the exenteration of orbit made in babyhood in age 1,5 years on an occasion retinoblastoma. Results: In first case, using Filatovs stem, it was succeeded to close the defects of fabrics of bridge of the nose, skate of nose and supraeyebrow region. In second case it was succeeded to form eyelids and conjunctival cavity and to make cosmetic eye prostheses. The repair flowed successfully due to good trophism of stem. Conclusions: Application of Filatovs stem can be the method of choice at closing of defects of fabrics with the broken trophism and forming of anatomic structures.
