AbstractSA.19.07 25-gauge transconjunctival sutureless vitrectomy Biewald E., Bornfeld N. Zentrum für Augenheilkunde des Universitätsklinikums Essen Objective: Recently the 25-gauge system gains more and more importance concerning the operative management of less complex vitreoretinal pathologies. We retrospectively report about our experience with the new sutureless system. Methods: We retrospectively report about the surgical outcomes of 101 cases of sutureless vitrectomy. Special interest was laid on the indication and postoperative complications. All patients were operated with the Alcon Accurus System. Results: Indications for the performance of 25-gauge vitrectomy were retinal detachment, vitreous bleeding, tumorbiopsy, macular hole, endophthalmitis, retrohyaloidal bleeding. Concerning postoperative complications we noticed transient hypotension in 14 patients but only one case required a suture to close. Four patient developed a retinal detachment postoperatively; other than that we noticed no complications. Conclusions: Transconcunctival sutureless vitrectomy seems to be a safe procedure concerning various surgically less complex vitreoretinal pathologies. There were a few cases of postoperative hypotension, which resolved spontaneously in all but one case. Other than that we noticed no complications related to the use of the 25-gauge system. Still the future role of the new system is not yet quite clear. With the development of further instrumentation and better equipment the indications for the use might become more widespread. To this point the standard 20-gauge system still seems to be the first choice for more complex vitreoretinal pathologies.
