Penetrating keratoplasty – indications and results

Objective: To identify the main indications for penetrating keratoplasty and analyze the results in patients referred for corneal transplantation at Medical Center for Sight “Pashev”, Sofia, Bulgaria.
Methods: Retrospective review of clinical records and follow-up of patients operated between September 2003 and September 2006 was performed. Demographic data were analyzed and indications for penetrating keratoplasty were identified. The period of observation was from 3 to 39 months.
Results: Fifty five patients were operated, 28 (50.91%) women and 27 (49.09%) men. The mean age of patients was 51.84 (range 18-79). Aphakic / pseudophakic keratopathy (n= 19, 34.54%) was the commonest indication, followed by keratoconus (n=18, 32.73%), corneal dystrophy (n= 7, 12.73%), leucoma (n=4, 7.27%), pellucid degeneration (n=3, 5.45%), corneal perforation (n=2, 3.64%), corneal abscess (n=1, 1.82%), and graft failure (n=1, 1.82%). Corneal graft remained clear in 43 patients (78.18%) for the period of observation. Graft rejection was observed in all patients with corneal scar after chemical injury and other corneal lesions with vascularization. Common complications were secondary glaucoma in 11 patients (20%) and cataract formation – 8 (14.54%). We diagnosed also suture complications in 3 patients (5.45%) and retinal detachment after combined procedure with intraocular lens suturing in 2 (3.64%).
Conclusions: An increasing trend in pseudophakic bullous keratopathy was noted as compared to indication analyzes from our previous studies. Secondary glaucoma and cataract formation were the most frequent postoperative complications. Our patient pool includes more complicated and emergency cases with poor prognosis as recently number of donor cornea is extremely restricted due to change in legislation and corneal recovery organization.
