Intraindividual comparison of an intraocular lens for Microincision Cataract Surgery (MICS) – 1-year follow-up

Objective: For microincision cataract surgery (MICS) specific intraocular lenses (IOL) have been developed. These lenses differ from conventional lenses. Thus, in a prospective clinical study a MICS-IOL and a standard acrylic lens were compared.
Methods: 25 patients received a bimanual MICS in one eye and a coaxial phacoemulsification in the contralateral eye, either a foldable acrylic lens for MICS (Acri.Smart 46LC-5) or, as a control, a standard acrylic lens (AcrySof SA60AT). One year postoperatively the visual acuity, the ocular aberrations (Hartmann-Shack-Sensor), the endothelial cell density and the posterior capsule opacification (POCOman) were measured.
Results: The postoperative visual acuity was 1.0±0.16 (Acri.Smart) and 1.04±0.15 (AcrySof), (p=0.53). There was no relevant rotation of the lenses. The higher-order aberrations (RMS-value; 5.5 mm optical zone) were significantly lower with the MICS-IOL (0.370±0.113 vs 0.501±0.106 µm; p<0.01). The differences in induced astigmatism (p>0.05) and in the development of posterior capsule opacification (14.1±20.9% vs 5.3±9.6% of the optical zone; p=0.39) were not statistically significant. No capsulotomy was performed. The endothelial cells loss was comparable (p>0.05).
Conclusions: The different operation methods for cataract surgery were comparable. One year postoperatively the implanted IOL proved to be stable and showed good optical properties. There was no relevant development of posterior capsule opacification.
