Viscocanalostomy: a possibility? A retrospective 5-year comparison

Objective: The purpose of this 5-year-study is to compare the results viscocanalostomy versus trabeculectomy for IOP-reduction and complication rate.
Methods: 199 patiens were treated by viscocanalostomy and 80 patiens by trabeculectomy. Retrospective results for IOP-reduction, visual acuity, success-, complication- and re-operation-rate were evaluated.
Results: By viscocanalostomy IOP droped from 21 mmHg for 23.8% after 1 year, by trabeculectomy from 28 mmHg for 46,4%. Visual acuity decreased small. The success rate (no re-op or local post-op treatment) was 36.7% by treatment with viscocanalostomy, by trabeculectomy 56.3%. After 15.2 month in mean 21.6% of our patients, who were treated by viscocanalostomy, needed a re-operation, by trabeculectomy after 3.9 month 17.5%. After treatment by viscocanalostomy 37.2% and after trabeculectomy 18.8% of our patients needed glaucoma eye drops.
Conclusions: Both treatments (viscocanalostomy and trabeculectomy) achieve a target-IOP below 18mmHg after one year. Viscocanalostomy is an efficient alternative for trabeculotomy with the right indication (for less IOP-lowering or high-risk-eyes).
