New developments in accommodation research

With the increasing importance of refractive procedures to correct ametropia there is also a growing interest in research about accommodation and surgical methods to treat presbyopia. According to the theory of Helmholtz which is widely accepted and has been experimentally confirmed in its relevant aspects, accommodation is based on the relaxation of zonular tension during the contraction of the ciliary muscle resulting in the molding of the lens into the accommodated form by the elastic lens capsule. Hence, hardening of the lens with advancing age constitutes the main factor in the development of presbyopia. Along with multifocal intraocular lenses and recently multifocal corneal ablations, intraocular lenses which are designed to restore a true accommodative change in the focus of the eye are increasingly used. So far, clinical studies have shown contradictory results regarding efficacy, predictability and stability of the achieved accommodation. For an adequate evaluation of the different methods comparative studies using objective measurement techniques are mandatory.
