Treatment of keratoconus with x-linking

Objective: The keratoconus is a progressive corneal disease, which is combined with a steepening, a myopic, a thinning out and an irregular astigmatism.
Methods: After removal of the epithelium (8mm Ø), riboflavin is seeped on the cornea and than irradiated with UVA-light (370 nm, 3 mW/cm2) at a distance of 1 cm for 30 minutes. 90 eyes from 67 patients (31,47±10,7 years) with a progressive keratoconus were operated with the collagen-crosslinking by using riboflavin/UVA-light. For the pre- and post-controls corneal topography, pachymetrie, tensio, refraction and the visual acuity testing were examined.
Results: The postoperative follow-up time was between 1 to 62 months (18,07±15,25). The K-values showed a significant progressive preoperative. Postoperative by 64.4% was the astigmatism reduced for 1,99±2,4 D (0,01 to 12,44). The best-corrected visual acuity increased by 0,62±2,23 lines. No side effects were observed.
Conclusions: The collagen-crosslinking at a keratoconus is an efficient method to increase the corneal stability and strength. This operation should be performed by every keratoconus progression, which would reduce the numbers of keratoplasties.
